Job Description
Position Summary:
The Lead Psychiatric Services Nurse (Lead PSN) is a member of the Department of Psychiatric Services who is responsible for providing direct nursing care to patients requiring psychiatric care within a family practice setting. They provide direct clinical supervision to other Psychiatric Services Nurses and to our Psych Community Health Workers, as well as serving on the DPS Leadership Team. They employ a flexible and humanistic management style that facilitates each team member reaching their full potential.
The Lead PSN understands the importance of integrated care. They are responsible for overseeing and helping to develop the tracking of screenings and medication monitoring needs using a population health model and panel management focus, facilitating patients meeting with their care team and carrying out their care plans, and providing education and support regarding psychiatric condition management. The Lead PSN is responsible for providing both psychiatric and primary care nursing to patients and should be familiar with both community psychiatric nursing and primary care nursing.
The Lead PSN applies the lens of trauma-informed care to their practice and must demonstrate a fundamental understanding of the roles of trauma and structural violence in people’s lives and the ability to work directly with highly-traumatized and structurally-vulnerable people to support their care needs and their path to healing and recovery.
DUTIES AND RESPONSIBILITIES:
• The Provider Leader collaborates with practice management and monitors patient populations and acts as consultant to other Medical Providers.
• Coordinates Quality Improvement (QI) projects in collaboration and coordination with FHCW Quality Council and works closely with Medical Director to achieve these aims.
• Day-to-Day Direction and Mentoring: Works with other Provider Leaders and their teams to ensure best practices for their special population are being utilized throughout health center.
• Assists in coordinating team-based QI projects.

Education And Experience:
Provider Team Leader is either a Physician, Nurse Practitioner, or Physician Assistant as their primary job description. Prior team leadership and/or management experience preferred.
LEAN, NCQA, or other similar quality management experience preferred.
